Transitions in Healthcare

This session brings together Dr Clare Mc Aleer, Prof Donal O’Shea and Dr Cara Dunne to explore important transitions across healthcare, from paediatric to adult services and medical care in gender transition to the transition from active management to end-of-life care.

By the end of this session, participants should be able to:

  • Recognise key considerations in identifying and managing the transition from active treatment to end-of-life care.
  • Develop an understanding of the clinical considerations involved in medical care during gender transition.
  • Explore the challenges and considerations involved in transitioning young people from paediatric to adult healthcare services, using inflammatory bowel disease as a clinical model.
  • Apply a more informed, patient-centred approach to managing significant transitions across different stages of healthcare.
